Alto – New Account Creation Gift Card Incentive – Terms & Conditions

Participant must create an Alto account by 11:59pm PST on 06/01/22 to qualify. Offer only open to new patients who: (i) do not have an existing Alto account; (ii) are 18+ years of age; (iii) have a delivery address within an Alto Pharmacy service area; and (iv) reside in the state of Texas. Offer not valid: (i) for existing or previous Alto patients; (ii) for beneficiaries of state or federal healthcare programs, such as Medicare, Medicaid, or Tricare; (iii) for new patients who reside outside of Texas; or (iv) if otherwise prohibited by law. Participant must provide the AltoNew code when creating an Alto account to qualify. No prescription transfer or prescription fill needed to qualify.  Offer code may not be distributed, sold, or redeemed for cash.  Offer code will not be replaced if lost or stolen and offer may not be combined with any other Alto offer or discount. Limit 1 per person. Attempts to redeem this offer more than once, by any means (including signing up for multiple Alto accounts), shall be void, and any participant engaging in such activity may be disqualified. Alto reserves the right to disqualify participant who submits a referral code while participating in any government or federal healthcare program or if Alto suspects fraud or abuse of the offer. Gift card will be sent via email to the address associated with participant’s Alto account within four (4) weeks. Alto shall not be responsible for any participant not receiving a gift card due to email filtering and/or an incorrect email address. Each gift card is issued by a third party and is subject to the terms and conditions imposed by its issuer. Alto reserves the right to modify or terminate this offer at any time and for any reason.
